Facility & Bus Usage
The person who submits the form is responsible for ensuring the facility or the buses used are cleaned and the bus filled with gas following your trip. After your bus usage is approved, contact the church office for keys and instructions. Before a person will be allowed to drive any church vehicle, you must fill out the proper paperwork prior to the event and a copy of your drivers license turned into the church office. For event details, please state the date, time, number of people expected, length of event, and any other details.
